Closed for lunch from 1:00p.m. - 2:00p.m.
Early morning and evening appointments are available by request.
Occasionally, the office is closed for Holidays or continuing education.
Appointments
Your scheduled appointment time has been reserved specifically for you. We kindly request a 24-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you may be subject to an additional fee. Insurance
As a special service to you, we assist in filing of insurance claims so that you might receive the full benefit available from your insurance coverage. We permit you to use your orthodontic benefit to lower your portion of the cost of orthodontic treatment, rather than you paying the full fee up front and waiting for reimbersement from the insurance company. This allows you the financial freedom of paying only your part of the treatment fee since we accept direct payment from your insurance company to our office. In relieving you of this financial burden, we allow ourselves to be very vulnerable to the insurance company; therefore, we have set some guidelines and limitations which must be recognized and adhered to. Past Due Accounts It is important to stay current with payments to keep the cost of treatment affordable for everyone! The billing party is responsible for keeping payments current. A $15/month late fee will be charged to all accounts 30 days past due. After 60 days of non-payment, you may be placed into a maintenance-only treatment status. At this time, no further treatment (except emergency care) will be provided until new payment arrangements are made or the account is brought current. If you have any questions, our courteous staff is always available to answer them. Orthodontic Charges The orthodontic treatment fee is for the total services rendered and is not directly related to the time required to complete treatment. We have extended the payment of this fee as a convenience to you; however, actual treatment time may be shorter or longer than the payment period. Our fee includes only the procedures performed in this office. Additional charges will be added for lost or broken appliances, missed appointments, or for uncooperative patients (after discussion with the responsible party). Our office reserves the right to discontinue treatment at any time and for any reason, including lack of cooperation or failure to adhere to the financial agreement. Broken Bracket Policy Extensive breakage can prolong treatment and affect the quality of care. Fortunately, most patients experience minimal breakage. After the third broken bracket, a $25 fee will be charged for each subsequent broken bracket. PLEASE CALL OUR OFFICE if you have a broken bracket so we may schedule you for a repair appointment. Bring all broken pieces with you. Transfer Policy If a patient in good standing relocates to another area, requiring treatment to be completed by another orthodontic practice, our fee may be adjusted to reflect fairly upon the services provided and the account may be credited accordingly. For the purposes of this calculation, your account will be pro-rated using a breakdown of 33% as earned at the treatment start plus the monthly charges based on the estimated treatment time. The number of broken appointments, any long absences from the practice without supervision, and the estimated time to finish treatment will also be taken into account. Dental Care During orthodontic treatment, you are expected to maintain your regular check-ups and cleanings with your family dentist every 4-6 months, or more if indicated. This will help to prevent cavities, decalcification, and gum disease. Ongoing poor oral hygiene, gingivitis, or progressive decalcification may result in discontinuation of orthodontic treatment.
